A Clinical Study of Pyelonephritis

Abstract
Acute pyelonephritis is an inflammatory process involving both the pelvis and the parent me of the kidney and it¢¥s diagnosis required docum:ntation of the coexistence of renal inflammatio and bacteriuria.
A clinical study was performed in 197 cases with acute pyelonephritis who were admitted to , the Department of Internal Medicine, Korea Un versity Hospital during the preceding ten years fro Jan, 1967 to Dec, 1976.
The results are as follow:
1) The ratio of male to female was 1 : 6 . nd the highest age incidences of acute pyelonephr itis was found in the 3rd decade (69 cases; 3.0%),
followed 4th decade (60 cases; 30.4%), and 5th decade (31 cases; 15.7%) in the order of fr:.uency and 58.4% of cases occured in reproductive female patients in 20-40 years of age.
2) The precipitating factors were recog ized in 42.2% of cases, and the major ones were pr;gnancy (22 cases; 11.3%), instrumentation(15 cases;0.6%), obstructive uropathy (14 cases; 7.1%), and Diabetes mellitus (10 cases; 5. 1%).
3. The common clinical symptoms were fever and chill followed by CVA pain nausea, frequency, dysuria, headache, indigestion and diarrhea in the Border of frequency.
45 Microorganisms were isolated by urine culture in 103 cases with the positive culture rate of 64.3 %. While Staphylococci were found more frequently than E. coli in male patients, E. coli was isolated in more than two thirds of female patients (69.8%).
